PARTICIPATION AGREEMENT:
I hereby give permission for my child(ren) to fully participate in the programs offered by The Organized Mind. I understand that this program may include a variety of physical activities, and while all necessary precautions are taken to ensure safety, these activities inherently carry a certain degree of risk of injury. I acknowledge and accept this risk and agree that The Organized Mind shall not be held liable for any injuries or costs incurred as a result of my child’s participation in these activities.
PHOTOGRAPH PERMISSION:
During the course of this camp, there may be occasions when photographs are taken of my child(ren). I hereby grant permission to The Organized Mind to use these photographs for marketing or promotional purposes, without any remuneration.
FAMILY HANDBOOK ACKNOWLEDGEMENT:
I confirm that I have been given access to, have read, and fully understand the Family Handbook provided by The Organized Mind. I agree to abide by the policies and procedures outlined in this handbook.
PAYMENT POLICY:
All payments are expected to be made by the due date as stipulated in the invoice. Should a payment be returned due to insufficient funds, or if no payment is received within 10 days of the due date, a late fee of $10 will automatically be levied on the account. Please note that no credits, prorated days, or refunds will be provided in the event of absences, regardless of the cause including, but not limited to, vacations, illnesses, or circumstances beyond our control, such as natural disasters.
REFUND POLICY:
We understand that plans can change, and we strive to accommodate these changes whenever possible. If a refund is requested at least 7 days prior to the start of the camp, we will process the refund, deducting a $25 administrative fee to cover the costs of the changes. However, if the request is made less than 7 days before the start of the camp, we will not be able to provide a refund, as outlined in our standard payment policy. Please consider these terms carefully when planning your child(ren)’s participation in our programs.
TRANSFER FEES:
In the event of a camp week transfer, a fee of $10 will be applied for each individual week that is transferred. This fee will be charged irrespective of the reason for the transfer.
